When you request a quote, we collect the information you enter in the form: such as your name, business name, email, phone, ZIP code, number of employees, and target start date.
How we use it
We use this information only to prepare and send your small group health insurance quote and to follow up about it. If you gave consent, we may contact you by email, phone or text about group benefit options. Reply STOP to opt out of texts at any time.
What we don't do
We do not sell, rent, or trade your information. We share it only with insurance carriers or administrators as needed to quote coverage, or when the law requires it.
Form processing
Form submissions are delivered to us by email through a third-party form service (FormSubmit).
